Tanner Health, Healthliant Ventures and CareTrack are pleased to announce an agreement for Tanner Health to deploy CareTrack’s remote care platform, a combination of patient care software and services. The platform increases engagement so Tanner Health’s patients will receive more consistent check-ins and an elevated customer experience. CareTrack predominantly focuses on remote patient monitoring (RPM) and chronic care management (CCM).
CareTrack’s services are expected to engage customers in a meaningful way, bolstering patient touchpoints even when they are not in an office or appointment setting. Some of the most important touchpoints will be monitoring vitals, supporting patients to follow their care plans and providing patients with a listening ear between appointments for questions.
Healthcare groups and health systems will also use CareTrack to enhance their experience as RPM services empower providers to make well-informed decisions. In addition to consistent patient support, business operations will be optimized, allowing for more consistent revenue growth.
CareTrack solutions can be personalized by each healthcare unit or group to enhance or focus on follow-up or patient support. It can also be used to track and improve a variety of other business needs. Tanner Health expects CareTrack to reduce the load on providers, enabling better-focused patient care.
